Workers Compensation
If you are injured while at work, you need to report your accident within 24 hours of injury to ensure coverage by Workers Compensation.
The attached form needs to be completed and turned in within 24 hours to ensure that the University is in compliance with our Workers Compensation carriers standards for timely reporting. Please email the form to EHS (email provided on form).
