Welcome to Alfred University's E-Form repository. To fill out one of the forms listed below and then upload, do the following:

  • To use this application, you must have Adobe Acrobat Reader installed on your computer. If you need to install this download here.
  • Right click on the form needed, click save link as or save target as (depends on your browser)
  • Popup will appear to save, select where you want to save
  • Click the save button
  • Save the file on your computer, keeping the default filename (e.g. 0014.pdf)
  • Go to where you saved the file
  • Double click on the file
  • Fill out the form
  • Instructions on including a digital signature
  • Save the file again on your computer, using the same filename
  • At the bottom of this page, click Choose file
  • Browse to the file on your computer, click submit. This will upload the form so the department can access it.
