- To use this application, you must have Adobe Acrobat Reader installed on your computer. If you need to install this download here.
- Right click on the form needed, click save link as or save target as (depends on your browser)
- Popup will appear to save, select where you want to save
- Click the save button
- Save the file on your computer, keeping the default filename (e.g. 0014.pdf)
- Go to where you saved the file
- Double click on the file
- Fill out the form
- Instructions on including a digital signature
- Save the file again on your computer, using the same filename
- At the bottom of this page, click Choose file
- Browse to the file on your computer, click submit. This will upload the form so the department can access it.
