General Incident Report Form
Use this form to report:

• Violations of university policies, including student code of conduct.
• Incidents including but not limited to alcohol misuse, drug use, violations of Residence Life or other campus policies, physical assault, organizational misconduct, disruptive behavior, or theft.
• Hazing incidents or information involving students, recognized student organizations, clubs, teams, etc.
• Bias-related incidents, whether you have experienced, witnessed, or been made aware of bias-related, discrimination, or harassment behavior toward another Saxon community member.
• Information about a student who is exhibiting behaviors that are of concern in relation to their personal, physical, or emotional well-being.

Title IX Incident Report Form
Use this form to report incidents involving alleged misconduct that involve or relate to:

• Sexual misconduct
• Sexual harassment
• Sexual assault
• Dating violence
• Domestic violence
• Stalking
• Retaliation

ACCIDENT & NEAR-MISS EVENTS

Use this form to report accidents or near-miss events to Environmental Health & Safety within 24 hours of the event.

• For major emergencies or accidents, notification must be made regardless of time or day (after normal hours, weekends, and holidays) to Public Safety 607-871-2108 or 911
• If an injury requires in-patient hospitalization or results in death, Public Safety or the supervisor must immediately contact Environmental Health & Safety.

GENERAL SAFETY CONCERN 
Use this form to report general safety concerns.

• General safety concerns
• Facilities & building issues, university grounds, roadways, parking lots, stairways.

REPORT A Maintenance or Facilities repair

This system provides the campus community with the following benefits:

• Improves the campus community's ability to communicate a work request to Facilities Services at any time of day or night.
• Provides real-time order status.
