Employee Account Activation
When an employee is first hired that needs an account, this form should be dropped off to the Human Resources Department in Greene Hall or faxed to 607-871-2318 after it is filled out. Additionally, Human Resources' secure file sharing location can be utilized to provide the form to them electronically. Do not email this form due to the sensitive information it may contain.
